Membership Application

People and organizations requesting membership must fill out this form in its entirety and then submit it. Please recognize that the PMTA Code of Conduct and Non-Disclosure Agreement must be read and reviewed prior to completion of this application.

  • Please provide a statement in paragraph form that captures your organization's mission, vision, and values, etc.
  • Please provide the name of the person who will be the primary contact with PMTA.
  • Please identify the services offered by your company.
  • Please outline how your organization ensures that students receive a quality learning experience. You may emphasize how content is delivered, how you select and monitor instructors, or how you follow up with students. You must include at least a 150-word description in order for your application to be considered.
  • Have your or your company delivered at least 200 hours of project management training?
  • Please insert two recent student evaluations (copy and paste into space provided).
